Josh took their disappearance very badly: the event caused him to drop out of college and triggered his existing chronic depression. As this became more and more severe throughout the year before the events of Until Dawn, he turned to Sam for support. While Dr. Hill does exist, in the game he is a hallucination of Josh: this hallucinatory psychiatrist is an embodiment of Josh's self-loathing and guilt, as seen when he aggressively chastises Josh for his ''game'' or berates him for not saving his sisters, even though he was physically unable to do so.
