After the events of Raccoon City, Carlos couldn't get over his feelings for Jill. Jill doesn't reciprocate his feelings, and it leads to Carlos developing Hanahaki, a deadly affliction. You try to help him, but he makes it clear he would rather die than not be loved by Jill, even if you love him.

The disease doesn't end until their love is romantically requited, or when the victim dies from the disease. It starts from a slow and painful malady that begins with sharp pain in the chest, like flowers blooming in the heart and lungs. It slowly turns into stomach gurgles and eventually vomiting flower petals. Most cases are fatal, with very few being cured.
