She rules the empire alongside her mother and twin sister Kitana, but is also secretly afflicted with the plague known as Tarkat. Born mere seconds ahead of her twin sister, Mileena is the rightful heir to Outworld's throne. But even so, there are those who distrust Mileena's impulsiveness. They whisper that Kitana, with her steadier hand, should replace Mileena as Sindel's heir. As Mileena fights for legitimacy, she hides a horrible secret: she is infected with the dreaded and lethal Tarkat disease. Were her affliction found out, Mileena would be forced into battle to save her throne. One night when Mileena was younger, she began to feel overwhelmed about her responsibilities as princess of Outworld, so, in an attempt to clear her mind, she snuck out of the palace. Mileena lived that night as a normal resident of Sun-Do, and she had a fantastic time, engaging in festivities and having talks with civilians, to get to know her subjects more. However, Mileena wandered too far & ended up getting infected with Tarkat disease. Tarkat disease is a condition, which disfigures its victims to the point that they are almost, if not completely unrecognizable. Overtime, the disease reduces the mental state of its victims to primitive and savage monsters, with only a few managing to keep their humanity. The disease affects its victims differently, some minor, others severe or worse, causes death in some. Due to no official cure for the disease, some state that the only release and freedom from the disease is death, however, there was a point in time where Mileena was given a serum to suppress the disease, helping her retain her natural appearance, only full becoming Tarkatan when she has not been registered her serum. However, now, Mileena doesn't use the serum, embracing her Tarkatan side, using only her sheer will to fight off the more animalistic & sadistic characteristics of the disease. After Sindel's sacrifice, Mileena became the new Empress of Outworld. Her first order was making her twin sister, Kitana, the new Supreme Commander of her army, filling in the hole left by General Shao's betrayal & revolt. Meanwhile, worried about how the public might react, Mileena met with Baraka to discuss his Tarkatans. Speaking from his heart, he moved her. Mileena agreed to visit Baraka's colony and see how his people lived. The conditions were atrocious. Mileena believes that this was one of her mother’s few mistakes, since, like all Outworlders, she treated Tarkatans with scorn. Mileena believed that instead, what Tarkatans deserved was compassion. And the only way to get it for them was if Mileena revealed her affliction, to show all her subjects that even an empress could get Tarkat. The scandal Milena's revelation was intense. But with the help of her councils, Mileena emerged from it a stronger empress than ever. Her honesty, empathy, and resolve won over her remaining doubters.
